Liability Release/Waiver Form 2024/2025
Welcome to K2 Studios LLC., & Komplex @ K2 in order for any individual to participate in any classes at K2 Studios LLC ,& Komplex @K2 Located at 4375 Prado rd. Suite 105, & 106 Corona , Ca, 92880 and also at the residence of 4225 Jameson Circle Corona 92881, Ca this form must be fully completed by all participants and parents. If the participant is under the age of 18, a parent or guardian must also sign this form.
* Admission to classes will not be accepted unless this waiver has been agreed to.

Waiver of Liability
I (or parent/guardian) recognize and understand the risks of physical injury inherent to dance, yoga, acro and dance training and I fully assume those risks. I hereby release K2 Studios LLC., its affiliate dance studios, event sponsors, employees and dance teachers & contracted instructors, Yoga Teachers, Acro Instructors or any other teacher or employee or Owners of k2 Studios LLC & Komplex @ K2 from all liability for injuries sustained or illness contracted while attending or participating in any dance classes, acro classes, rehearsals, workshops, conventions or performances or viewing. I agree to indemnify, defend, and hold harmless K2 Studios LLC., & Komplex @ K2 its affiliate dance studios, event sponsors, employees, volunteers, and dance teachers and contracted instructors for liabilities, cost and judgements arising from acts of omissions committed by me or my child or friends and family that I bring on to the K2 Studio LLC or Komplex @K2 Property which result in injury or damage to any persons or property. I agree to take full responsibility for any adult or child, or persons that I bring into the studio. This also releases K2 Studios LLC& Komplex @ K2 of any fault, wrongdoing, or Liability from slip and fall accidents or injuries sustained from falling, slipping or tripping in the studio while viewing, dancing or any other activity or non-activity while on the K2 Studios LLC & Komplex @ K2 property. This Liability waiver also releases Liability from all prop activity or dancers who fall while using a prop or dancing with a prop. If there is damage or injury that occurs to the property or persons, I will take full responsibility and I will not hold K2 Studios LLC & Komplex @ K2 Liable for any damages, Lawsuits or financial Liability.
AGREEMENT TO PARTICIPATE AND LIABILITY WAIVER - I understand acrobatics and dance and other sports activities involve risk and possible injury, including but not limited to paralysis, death, emotional distress, or damage to myself, to property, or to third parties.
I understand that it is my responsibility as an adult participant or parent not to participate or allow participation if there are any physical, emotional, and behavioral or other problems that might compromise safe involvement. I expressly agree and promise to accept and assume all of the risks existing in this activity as outlined above.
My participation or my child’s participation in this activity is purely voluntary, no person(s) are forcing me or my child to participate and I elect of my own volition to participate or have my child participate with full knowledge of the inherent risks involved.
I hereby voluntary release, forever discharge, and agree to hold harmless and indemnify K2 Studios LLC& Komplex @ K2 from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in anyway connected with my participation or my child’s participation in this activity. Should K2 Studios LLC & Komplex @ K2 or anyone acting on their behalf, be required for any reason to incur attorney fees and costs to enforce this agreement, I agree to indemnify and reimburse K2 Studios LLC & Komplex @ K2 for such fees and costs.
PARENT RESPONSIBILITY TO SUPERVISE - When I visit K2 Studios LLC. & Komplex @ K2 and am involved in any related activity involving parental presence or participation, I understand and accept the responsibility, and any associated liability, of constantly supervising, controlling, and restricting activities as necessary to assure safety of the children I bring and myself.

VALID DATES - These agreements, waivers, and authorizations will remain valid and in force as long as and whenever my child, myself, or any family member participates in any activity at and/or with
K2 Studios LLC. & Komplex @ K2
By signing this document, I acknowledge that if anyone is hurt or property is damaged during my participation in this activity, I may be found, by a court of law, to have waived my right to maintain a lawsuit against K2 Studios LLC& Komplex @ K2 on the basis of any claim from which I have released them herein. I have had sufficient opportunity to read and fully understand this entire document and I agree to be legally bound by its terms.
Property Protection
I understand and agree that it is my responsibility that it is my sole responsibility to safeguard my personal property while attending or participating in any classes, rehearsals, workshops, or performances. I hereby release K2 Studios & Komplex @ K2 its affiliates, event sponsors, employees, volunteers, and dance teachers from all liability for loss or damage to my personal property while attending or participating in classes, rehearsals, workshops, or performances. I also agree to abide by any rules, regulations and policies set forth by K2 Studios LLC. & Komplex @ K2

Photo Release
I further acknowledge that K2 Studios & Komplex @ K2 reserves the right to use photographs and videos with or without audio taken during classes, rehearsals, workshops, performances or other affiliated events for the purpose of liability, instruction, advertising and promoting K2 Studios & Komplex @ K2 and its programs.

Medical Attention
In the event of a medical emergency, I hereby authorize K2 Studios & Komplex @ K2 to make the necessary arrangements to transport myself or my child to a medical treatment facility as necessary. All such transportation and medical treatment will be at my sole cost and expense.

Acknowledgement of Waiver
I (or parent/guardian) Acknowledge that by agreeing to this document I have a full understanding and agreement of the content within this waiver. I agree to hold K2 Studios LLC, & Komplex @ K2 and its affiliates, harmless of any medical costs that may arise as a result of injury to me or my child. Furthermore, I state that there are no health-related reasons or problems which preclude or restrict my or my child’s participation in any activity.
